Varicose veins are a direct result of the reversal of flow in the superficial vein of the legs. The most common underlying issue is damage to the one-way check valves. Damaged valves cause a reversal of flow or reflux blood back down the leg, with pooling in the leg veins. Over time the vein walls dilate, elongate and bulge.
Varicose veins cause heavy achy legs, leg swelling and burning/stinging. Restless Leg Syndrome can be a symptom of vein disease as well. Left untreated, it can lead to bigger issues with blood clots (DVT/SVT), skin changes and venous ulcers.

Ambulatory phlebectomy is Dr. Kenneth Harper’s preferred treatment for bulging varicose veins because of its success in relieving the visible varicose veins and symptoms in a one and done procedure. However, offering ultrasound-guided sclerotherapy for bulging varicose veins is a part of the VSS toolbox for vein care.
For some patients with complex patterns of varicose veins or recurrent varicose veins, sclerotherapy is better. When varicose veins recur after previous treatments, they typically form because of the formation of new blood vessels. These ‘neo’ veins are delicate, soft and tortuous. For this reason, ultrasound-guided sclerotherapy, often with foam, is our preferred treatment. A proprietary foam sclerosing solution is used to treat these varicose veins. The veins close them through the scarring or sclerosing process over time. The blood supply is then rerouted to healthier blood vessels near the treated veins, leaving your legs smoother and your veins less prominent.
Your candidacy for sclerotherapy for varicose veins begins with a comprehensive consultation. During your varicose vein removal sclerotherapy procedure, your diseased bulging varicose veins are injected with an FDA approved sclerosing solution, with ultrasound guidance to confirm the best placement in the affected veins. The treated vein segments are manually compressed for a prescribed amount of time, and then your legs are wrapped in compression hose. You’re encouraged to walk as soon as you are able.

Because varicose veins are larger, they require more time than spider veins to show definitive improvement, up to 3-4 months. For some patients, multiple treatments may be necessary to achieve optimal results. Overall, your varicose veins should show a reduction in size and relief of symptoms.
The use of compression hose speeds healing and improves response rates to sclerotherapy, but you may resume your normal routine immediately while avoiding strenuous activity and sun exposure for 2 weeks.
Side effects are mild and may include trapped blood or tender nodules developing in the treated veins. These pockets of blood may be evacuated with a needle during follow up treatments. Other side effects may present as swelling, bruising, allergic reaction to the sclerosing solution, and hyperpigmentation. More serious risks that possibly require treatment are superficial and deep vein thrombosis (DVT), pulmonary embolism, and transient neurologic findings. If you experience shortness of breath or chest pain, notify your provider immediately.
